Our Sliding Fee Program offers a discounted rate on medical and dental services offered at our facilities. In addition to our Sliding Fee Program, our facilities provide convenient parking and easy access for patients with a physical disability, as well as materials in both English and Spanish, and interpretive services.

Patient Education

SouthEast Lancaster Health Services partners with Harrisburg Area Community Colleges’ Nursing Program at our South Duke Street center to offer free patient education on many health topics/conditions.

 

Social Work

Our Social Work department connects patients’ medical and dental experiences with the resources needed to support overall health, including:

  • Coordination of care
  • Connection to community support services
  • Health education
  • Support for refugees
  • Patient advocacy
  • Enrollment for Insurance Marketplace, Medicaid, and CHIP

 

Wellness Circles

Wellness Circles offer individual care, group education, and peer support to patients experiencing a similar diagnosis. SouthEast Lancaster Health Services offers classes for expectant parents, new parents and their babies, diabetes management, asthma management, weight management, men and women’s health, and more!

Coordinated Care

Your healthcare team will work to support your medical needs by obtaining a complete medical history, including any records of care outside of the practice, to help create the best plan of care.

Accessible Services

Patients can access the network 24 hours a day, 7 days a week by calling the main number for each site. During regular office hours your call will be answered by a patient service representative. After hours, your call will be handled by our answering service. You can also electronically send a message to your care team anytime through the patient portal.
